2. Download your files instantly.
   After checkout, you’ll receive download links right away. Templates are delivered as Microsoft Word documents (.docx) inside a ZIP folder so you can save them on your own secure system.
3. Customize the template for your patient.
   Open the letter in Word, add your logo, and fill in the bracketed fields like [PATIENT NAME], [DATE OF SERVICE], [TOOTH #], and specific clinical details. Your team can adjust the wording to match each payer’s guidelines.
4. Attach supporting documentation.
   Use your normal process to attach perio charting, radiographs, intraoral photos, and clinical notes. The templates clearly reference typical documentation so reviewers can follow your logic easily.
5. Submit through your usual channel.
